Cost for services (Uninsured/Private Pay):

• Initial Assessment: $175 (75 min)

• Individual or Family Session $125 (55 min)

• Group Services: $35 (30-45 min)

• Additional Report writing: $125 (60 min, billed in 15 min increments)

• BCBA services: $50 per 15 minute increments

• BCaBA services: $40 per 15 minute increments

• RBT services: $30 per 15 minute increments

• Intern Mental Health Services: $35/session (may bill some insurance)

Sliding Scale Cost:

• Due to set reimbursement agreements with insurance companies, Inclusive Options, LLC is not able to offer a sliding fee scale. Please discuss any difficulty with payment with your therapist. 

• We do offer a lower service rate with one of our Indiana University- Columbus Interns.

Co-Payment: Co-payments are expected at time of service and will be verified with your primary insurance provider.

Balance Billing:

• Inclusive Options, LLC does not participate in balance billing, meaning you will not be charged for any remaining balance from a service that your insurance has already paid. 

• If your insurance denies payment for a service, you will be billed for that service at the outlined rate above as they did not make any payment of that service.

Cancellation, Late, No Show  Policy: 

• Appointments may be canceled via telephone up to 6 hours prior to the appointment time without an additional fee. 

• Clients who cancel with less than the required notice and do not show for their scheduled appointment will be subject to a $35 fee. 

• Emergency cancellations will be addressed on an individual basis. 

• Clients who are more than 15 min late may be required to reschedule.

Court Appearances, Reports & Request for Records Costs:

Additional charges may apply for the preparation of reports, preparing of records for third party entities and for attending court hearings depending on the nature of the request(s) and at the full discretion of the therapist. The standard hourly rate will be applied for the generation of reports and preparation of records for third party entities with a minimum of a ¼ hour charge with additional time to be charged in ¼ hour increments thereafter. Most insurance companies will not reimburse clinicians for report writing or the preparation of records. Fees associated with these services will be the responsibility of the client with payment for these services payable at the time of service. Reports are typically recommended in lieu of formal court appearances.
